A mistake-proofing device is any mechanism that either prevents a mistake from being made or makes the mistake obvious at a glance. The ability to find mistakes at a glance is essential because, as Shingo writes, "The causes of defects lie in worker errors, and defects are the results of neglecting those errors.  It follows that mistakes will not turn into defects if worker errors are discovered and eliminated beforehand." Most mistake-proofing devices are physical objects which can be photographed.

Dr. Grout has been researching, documenting and teaching Mistake-proofing for 20 years.  Learn about this technique for eliminating defects, improving quality, and streamlining work. He will discuss mistake-proofing within the specific context of health care, including lots of examples.

About the Presenter: John Grout is dean of the Campbell School of Business at Berry College, Rome, Georgia, and the David C. Garrett Jr. Professor of Business Administration.  John has taught courses in operations and service management, quality, statistics, process improvement, quantitative methods, and materials management at Berry College for 15 years. He holds a Ph.D. from Penn State.  In 2004 John received the Shingo Prize for his paper, The Human Side of Mistake-Proofing with Douglas Stewart.   In May 2007, John’s book "Mistake-Proofing the Design of Healthcare Processes" was published by the Agency for Healthcare Research and Quality and is distributed free to the public.
